We strongly encourage you to have your employees fill out the questionnaire
online.

| Cost efficient, fast and easy! If your employees have access to
the Internet from their own workstation, general access computers or at
home, it’s the obvious solution – no expenses related to photocopying the
questionnaire or transcribing the responses, no handling of questionnaires
and envelopes. If your employees do not have access to the Internet, paper
is the other option. |
If employees do not have access to Internet, paper is
then the alternate solution. |
If you choose the paper option, we will provide you with original
documents for you to photocopy and distribute. Please note that a data
processing fee between of aproximatively $3.00 will be charged per paper
questionnaire.
