How to Register

BEFORE MARCH 31^st:

1. Fill out the Registration Form and submit it on line
2. Sign and return the Waiver of Responsibility

As needed, contact us with any questions related to:

• Determining your target response rate
• Customizing the questionnaire for your organization's specific needs
• Choice between "online or on paper"

Once your registration is complete, you will have access to all the information you need to proceed and the tools you need to administer the survey.
